Faith Formation Registration 2022 – 2023

Registration has been extended to June 19, 2022.

Students must be registered no later than June 19 in order to receive materials on time and receive program updates, including sacrament preparation requirements.

Please Note:  We will email all families in the month of June to confirm receipt of your registration, your tuition payment instructions, and Year 1/Year 2 placement (if your child is in a sacrament preparation year).

The form takes approximately 10-15 minutes to complete and you cannot save and come back later to finish.  If you do not complete all the information, your responses are not saved.
